Paperwork for Athletic Clearance:

Physical on record with Green Tech within the last 12 months. (Physical were formerly required every 20 months but are now required every year)
Sport History update with COVID 19 questions filled out. (If your son has tested positive for COVID 19 since March 2020 you must get written consent from his medical doctor stating that they are cleared to participate.)
Parent participation waiver.

If your son makes the team there will be more paperwork that will need to be reviewed dealing with COVID policies and Procedures for during the season.

Student Athlete Academic Policy which is in the handbook will be followed during the season as well.

Three steps for completing the online form:

Step 1: Fill out the form online (you have to agree to Adobe's Terms of Use and press the blue "Continue" button to start filling out the application). If a field doesn't show up as a fillable field, skip it, it will autofill on completion.

Step 2: Press "Click to Sign" Button once complete

Step 3: Verify your email address by opening link sent by Adobe to the email address used in the application (it may be in your "Junk" or "Spam" folder). Once you do this your application will be submitted and you will get a copy of your filled application sent to your email address.
